This poster highlights how the Montgomery County Babies Born Healthy Program leverages community partnerships to improve maternal and infant health outcomes. Collaborations with social support agencies, community organizations, and healthcare providers enhance access to prenatal education, mental health resources, social services, and tangible goods. By centering the voices and experiences of Black mothers, Babies Born Healthy collaborates with partners to strengthen family well-being.
